Imagine your breath going into your shoulders and releasing any remaining tension. Notice how your shoulders feel when they’re tight compared to when you let go of that tension. When doing these exercises try not to tense any other parts of your body except for the specific muscle group you’re working with. Breathe deeply, gently and smoothly letting your stomach expand as you breathe in. At the end of the inhalation let all the tension go quickly and then continue to breathe into your abdomen for two more slow deep breaths. Hold that tension as you breathe in and let go of the tension all at once as you breathe out. Breathe for two more complete breaths as you bring your attention to your buttocks. Breathe for two more complete breaths as you feel the physical sensation of release in your feet and thighs. Breathe in and out for two breaths as you scan for any areas remaining in the body that are still tense. Again, if you’re doing this exercise on the floor, roll onto your right side first; push yourself up to a sitting position with your arms. Progressive Muscle Relaxation: Short Version In situations where there’s not enough time to go through an entire Progressive Muscle Relaxation exercise, you can practice a shortened version. Mindfulness and the Body • 81 Start by tensing and relaxing in sequence: H • Facial Muscles • Hands, Arms, Shoulders, Neck all at the same time • Stomach, Chest all at the same time • Buttocks, Feet, Legs all at the same time Don’t forget about your breath. The key is to inhale while you tense the muscle areas, and exhale to release the tension. Remember to take two more complete breaths afterward, feeling the physical sensation of relaxation in the designated muscle group. The following is an exercise that will help further develop the practice of Progressive Muscle Relaxation. Practice Progressive Muscle Relaxation for ten to fifteen minutes in the morning and/or evening, whatever works for you. Alternatively, do a Progressive Muscle Relaxation practice once, twice or as many times as you want to in a given session. If you’re able to, take a break at your desk, or during television commercials, or when you’re at your computer and progressively tense and relax your muscles. Gradually your body will learn the difference between these two states and it will become easier and easier to find and release tension. How to Deal With Physical Pain The real test of body-awareness comes when you have to deal with physical pain. You have previously learned how slow, deep, smooth, belly breathing, with a prolonged exhalation, lessens stress. If you can direct the breath, like a laser, into the area of pain, this will allow that area to relax and feedback to create additional mental relaxation. Remember when my patient Mika was having abdominal cramps and she believed that she had cancer? If you’re having chest pain, it’s hard to believe it could be anything but a heart attack even when you’ve been given the all clear and you know your heart is fine. You probably experience more pain from the anxiety around the story you tell yourself than from the original physical pain. Bringing awareness to the story can help to lessen the secondary mental pain that accompanies the actual physical discomfort. As you’ll learn in the next section, identifying the process of original sensation to subsequent story can help take you out of the storyline and lessen the stressful drama that you create in your mind. As you bring awareness to the story your mind is telling, step back and inquire about the truth of what your mind is saying. Try to experience the “pain” as the changing physical sensation that it really is. As you bring your continual attention to the area, the Mindfulness and the Body • 83 sensation of pain will start to change, perhaps lessen and ultimately your mind will be drawn elsewhere.

From Arraignment to Conviction: Thought Court We base our technique called Thought Court on the principles of cognitive therapy. Beck, who discovered that changing the way people think changes the way they feel. Many studies attest to the fact that cognitive therapy works very well to alleviate anxiety and depression. We give you examples of Thought Trackers in this section, but for more information, flip to Chapter 4. Thought Court is a process of indicting the accused thought (the one you pinpoint in your Thought Tracker) and then bringing it to trial. As the defense attorney, you present the evidence that supports the validity or accuracy of the thought. In other words, the defense claims that your thought is true and isn’t culpable for your anguish. On the other side, you, as the prosecutor, lay out a case demonstrating that the thought is actually guilty of distortion and therefore has caused you unnecessary emotional distress. If you find the thought guilty, we give you ways to replace or rehabilitate your thought. Most people learn better through stories and examples than through laborious explana- tions. With that in mind, we help you master the process of Thought Court by presenting a case example in the next section. Then we give you the chance to put your thoughts on trial, and in case you need more help, we follow up your practice with more case examples. Examining a sample case in Thought Court Jeremy is a good looking 23-year-old personal trainer who takes pride in his healthy lifestyle. He’s known at the gym for the colorful, long-sleeved T-shirts that he always wears. Jeremy gets more than his share of attention from women, but he never gets involved because he has a secret: He was seriously burned as a child, and his chest and arms are deeply scarred. Jeremy has never had a serious rela- tionship; he believes any woman seeing his body would recoil in disgust. Rather than face rejection and ridicule, he locks himself away in solitary confinement. His com- bination of fear and yearning motivates him to see a therapist, and he manages to tell his therapist about his lifelong secret. Jeremy’s therapist suggests that he start examining his thoughts with a Thought Tracker (see Worksheet 6-1) and then take his thoughts to Thought Court. Chapter 6: Indicting and Rehabilitating Thoughts 79 Jeremy’s most malicious thoughts: 1. Next, his therapist suggests that Jeremy put the first of these thoughts on trial using a worksheet (later on, they address his other malicious thought). As you can see in Worksheet 6-2, Jeremy writes down the malicious thought first and then in one column defends the thought by listing all the reasons, logic, and evidence he can muster to support the case that the thought is true. In the other column, Jeremy attempts to prosecute the thought by demonstrating that it’s false. You can kill a man with large doses of Podophyllin, Lobelia, Jalap, and a hundred agents of like character, when small doses would not kill, and might be medicinal. The dose may be large enough to be poisonous, and then the size of the dose will be the only element of danger, but there is another consideration of more importance. Those agencies that we call remedies exert an action upon the body, and change one or more of its functional activities. If we give a drug to a healthy person, it produces disease, and it is because it thus acts upon the body that it becomes a remedy - an agent that had no such action would be useless. To make this agent a remedy, however, it is essential that there should be a functional wrong of the part upon which this agent acts, and that its action opposes the wrong of disease, and favors the return to health. If now we mistake, and give a medicine to influence a functional wrong that does not exist, then we are poisoning our patient - it may be slowly, but the influence is nevertheless poisonous. To illustrate, we find a condition of the system in some malarious diseases in which Quinine is tolerated in large doses, and is curative. We find cases in which patients can take large doses of the Bromides with safety and benefit for a long time, but in others the remedies are poisonous in moderate doses. I have seen serious results from the use of Iodide of Potassium for a long time, as I have from other remedies given by rote. And yet we find pickles greened with Copper as an article of food on many tables for years. My use of Copper has been attended with the happiest results, and I use it with quite as clear a conscience as I use Iron, but I only use it when Copper is wanted. I have been censured for the use of Bismuth, because it is not a constituent of the body, and yet I have never seen the harm following this agent, that I have from Podophyllin. It has been clearly demonstrated within the past two years, that Phosphorus and Arsenic are very closely related chemically and medicinally. That Phosphorus exerts a very similar influence upon the skin, and may be used in place of Arsenic in skin diseases. Yet Phosphorus as Phosphorus is by far the most difficult to use, the most unmanageable, and the most likely to poison the patient, yet none of these rabid Eclectics object to the use of Phosphorus? I object to the use of Antimony upon the same grounds, to the use of the lancet, the blister, harsh purgation, the entire class of antiphlogistics, from what ever source obtained. I base my objections upon principle, and not because there is a prejudice against these things. If I believed in the necessity of antiphlogistics, as some do, I should employ bloodletting and Mercury as the principal means, because they are the typical antiphlogistics, and will destroy life faster than any thing else. The perfection of specific medicine is found in those cases, in which the entire series of functional lesions is removed by one drug. Possibly medicine will never attain such perfection; it is possible we have quite as much knowledge now as we ever will have, but as we have some marked examples of this, it is reasonable to conclude that time will develop more. In studying the specific action of Tinctura Ferri Chloridi we found some singular but well attested facts. We found that this remedy was specific in a majority of cases of erysipelas, and that in many of these its action could not be accounted for by the older therapeutic doctrines. Here is a disease presenting all the functional lesions of the severest zymotic fever. A hot, dry skin, temperature of 106°, pulse 120 per minute, small and hard, mouth dry and parched, tongue brown, sordes on teeth, delirium, and an unpleasant local inflammation constantly spreading. We commence the administration of Tincture of Muriate of Iron in doses of ten drops. Before the second dose the effect is noticed, and within twenty-four hours, the pulse has fallen and become more natural, the temperature is reduced, the skin is soft and moist, the delirium has passed away, the patient has slept and is conscious, the mouth is moist, tongue cleaning, and the local inflammatory process arrested. In a few days the patient is convalescent, and yet but a single remedy has been given.
